- Complete your appointment paperwork electronically You will be receiving an email from the telederm team with a link to complete all necessary forms through HelloSign. Please complete all paperwork at least 24 hours in advance of your visit.
- Email or upload photos of your area of concern – to your physician (encrypted) Your clinician will need photos that show your areas of concern. Photos can be uploaded and attached to your intake form via HelloSign. The last question on the form will give you this option. If you have any difficulty uploading your photos, please call the office where your appointment is scheduled.

4. Phone or Video Chat Visit?
Depending on your clinician, your visit will either be by phone or by video chat. Our Telederm team will let you know when your appointment is scheduled.
If your visit is by phone, you are all set. There is nothing more to do.
If your visit is by video chat, our team will email you a special web link for your visit along with instructions on how to check in for your visit. Read through the instructions and check your link to make sure you are familiar with the process before your appointment.

What If I Need an In-Office Procedure or Skin Biopsy?
If your doctor determines that an in-office procedure is needed, our staff will schedule an in-person appointment as soon as possible.
